  • Salary Range: $25.75 - $26 per hour
  • Job Type: Full-time/Onsite
  • Work Schedule:

    Typically, 8:30am – 5pm Monday – Friday.

Who is Youth Link?

Youth Link is a recognized leader in the Twin Cities and beyond for our work to build pathways of opportunity for homeless youth and youth in crisis ages 16 – 24. Our model is based on providing support through outreach, handling basic needs, intervention, and aftercare. Our youth-focused team provides innovative, evidence-based services that guide youth on a holistic level toward futures of education, employment, health and wellness, housing stability and aftercare.

We are looking for a Chemical Health Case Manager to join our team and help us advance our mission. As a Chemical Health Case Manager, providing comprehensive case management services to individuals and families affected by chemical health issues, such as addiction and substance use disorder. You will provide clients with individualized care plans and coordinate services with other providers to ensure the best possible outcomes.

You will also be responsible for assessing client needs, advocating for clients, and providing education and resources to assist in their recovery. If you have a heart for helping those in need and are passionate about chemical health, we would love to have you join our team.

Required Experience, Licensure, Certification for entry into position
  • 1 year of case management experience
  • 2 years’ experience working with clients experiencing homelessness
  • 2 years’ experience with community engagement
  • 2 years’ experience working with clients going through chemical health issues
  • Must be able to come to work on time, for all regularly scheduled work shifts
  • Valid MN State Driver’s License and insurance
Preferred Knowledge, Skills and Competencies needed to successfully perform the work:
  • Knowledge of chemical health, substance use disorders, addiction services, recovery principles, and harm reduction practices.
  • Ability to assess client needs, develop individualized treatment plans, and provide effective case management services.
  • Knowledge of community resources and the ability to coordinate care, provide referrals, and advocate for clients.
  • Ability to provide crisis intervention, de‑escalation, problem‑solving, and support during challenging situations.
  • Strong understanding of trauma‑informed, youth‑centered, and culturally responsive service delivery.
  • Excellent interpersonal, verbal, and written communication skills with the ability to engage diverse populations.
  • Strong organizational, time management, and multitasking skills with the ability to manage competing priorities.
  • Ability to work independently and collaboratively within a multidisciplinary team environment.
  • Commitment to Diversity, Equity, Inclusion, and creating welcoming, inclusive environments for all participants.
  • Ability to build and maintain professional relationships and boundaries with youth, co‑workers, partners, and community stakeholders.
  • Understanding of professional ethics, confidentiality requirements, and appropriate documentation standards.
  • Proficiency in Microsoft Office applications and the ability to learn and utilize electronic databases and case management systems.
  • Ability to positively represent the agency and uphold Youth Link's mission, values, and guiding principles in all interactions.
