The Developmental Disabilities (DD) Waiver Case Manager is a full-time, on-site role based in Ramsey County. This role is responsible for coordinating and managing DD Waiver services for individuals with developmental disabilities, ensuring they receive person-centered, culturally responsive supports that promote independence and quality of life. Daily tasks include conducting intakes and assessments, developing individualized service plans, coordinating with service providers and community resources, and monitoring service delivery for compliance with state and county requirements.

The Case Manager will maintain accurate documentation, complete required reports, and participate in interdisciplinary team meetings and case consultations. The role also involves collaborating closely with individuals, families, guardians, service providers, schools, and community partners to advocate for client needs, provide education on available services, and support early intervention and long-term planning.

• Strong skills in Communication, including active listening, clear documentation, and the ability to build trusting, professional relationships with individuals, families, and providers.
• Knowledge of Psychology and human development, with the ability to understand the needs, strengths, and challenges of individuals with developmental disabilities.
• Experience or familiarity with Early Intervention and Developmental Disabilities services, including DD Waiver programs and community-based supports.
• Ability to deliver or support Training related to services, rights, and resources for individuals, families, and community partners.
• Bachelor’s degree in Social Work, Psychology, Human Services, or a related field; with professional experience in developmental disabilities as a case manager or direct service provider.
• Experience working with culturally diverse communities and a demonstrated commitment to cultural responsiveness, equity, and inclusion.
• Knowledge of local and state regulations related to DD Waiver services, case management, and social services best practices.
• Strong writing, organizational and time-management skills, with the ability to manage a caseload, meet deadlines, and maintain detailed records.
