Our programs focus on natural supports, experiential learning, education, and a therapeutic approach with expertise in trauma‑effective care.

$23.00 per hour

Family Wise is hiring a Wraparound Care Coordinator to facilitate a team process to give families voice and choice, with the goal of helping young people with behavioral health issues to live in a stable family or community setting.

• Engaging children and families involved with multiple systems.
• Developing and facilitating family‑centered teams.
• Conducting strengths and needs assessments.
• Developing and implementing individualized Wraparound plans.
• Collaborating with community partners.
• Modeling appropriate social skills for parents and their children.
• Communicating effectively with county workers.
• Supporting parents to improve life skills to be able to manage new needs as they arise.
• Participating in training, credentialing, and evaluation processes for High Fidelity Wraparound.
• Participating in monthly group coaching meetings and case consultations.
• Participating in other trainings as applicable.
• Maintaining client files to include assessments, program plans, client progress, and assessments by other professionals, intake forms, client outcome data, referrals, and other pertinent information.

• You have a bachelor’s degree in social work, psychology, parent education, criminal justice, or a related field, or you can demonstrate equivalent knowledge gained through alternative courses of study and life experience.
• You have knowledge and experience working with culturally and racially diverse populations in lower socio‑economic conditions.
• You can speak multiple languages, which is highly valued.
• You demonstrate strong organizational skills, allowing you to manage multiple priorities and meet deadlines.
• You understand the importance of customer service and professionalism.
• You showcase skills in conflict management.
• You are computer literate, with proficiency in MS Office.
• You can work both independently and as part of a team.
• You use good judgment in recognizing the scope of your authority.
• You can travel to client sites within a 15‑mile radius of the office.
