Position: Social Worker (Internal Hennepin County applicants only) - Youth Justice Behavioral Health

Social Worker

The Behavioral Health area of Human Services Public Health is seeking a Social Worker to provide Children's Therapeutic Services and Supports (CTSS) to children and youth with mental health needs referred through the Mental Health Center, a Certified Community Behavioral Health Clinic. The CTSS Social Worker will provide individualized, measurable interventions based on an Individualized Treatment Plan and provide skills-based interventions to support children and youth in returning to age-appropriate functioning.

They will work collaboratively with families, schools, and community providers to ensure coordinated, effective care. Responsibilities also include documenting services in Epic, monitoring progress, and adjusting interventions based on clinical need. This position works closely with the Mental Health Center, Long-Term Services and Supports, and other Hennepin County partners to support youth across systems.

Behavioral Health is an innovative and growing service area within Hennepin County's Human Services department. We provide a wide range of mental health and substance use services to residents, including children, adults, and individuals involved in the criminal justice system. Our team includes over 300 staff to include: social workers, psychologists, program managers, supervisors, planners, and others—working collaboratively to achieve system goals of expanded access, upstream support, and person-centered care.

Location and hours:
This position is hybrid and will be performed primarily in the community but also occasionally on-site at the Mental Health Center at 2215 Lake Street, Minneapolis, MN 55407, and remote as job duties require. Typical work hours will be Monday through Friday, 8 a.m. to 4:30 p.m., and occasional evening hours will be required. While this position is designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin.

About this position type:
This is a full-time, benefit earning position. You must be a current regular or probationary employee of Hennepin County to apply for this position. Limited duration employees are not eligible. This position is internally classified as a Social Worker.  to view the job classification specification. In this position, you will:

  • Provide comprehensive behavioral health services and direct skill services to children and youth with mental health needs that are referred by the Mental Health Center.
  • Teach coping and psychosocial skills to manage mental health symptoms.
  • Complete required documentation in Epic such as person-centered case notes, Individual Treatment Plans, support plans, functional assessments, and track progress.
  • Provide care coordination by building partnerships with individuals served to identify and advocate for their needs while helping to reduce any barriers or racial disparities while accessing services.
  • Work collaboratively with internal and external partners and providers to facilitate the goals and services in the individual's best interest.

Need to have:

  • One of the following:
    • Bachelor's degree or higher in social work, psychology, sociology, human services, or an equivalent field.
    • Licensure as a social worker by the Minnesota Board of Social Work.
  • A valid driver's license, the ability to obtain a Hennepin County driver's permit, and daily access to reliable transportation sufficient to travel to a variety of Hennepin County community locations, to visit clients in residential settings, and to occasionally transport clients.

Nice to have:

  • Experience:
    • Working with children and youth with mental health needs, developmental and/or intellectual disabilities, physical disabilities, substance use disorder, and/or medical conditions.
    • Teaching emotional regulation and social skills.
    • Using Epic electronic health records system.
    • Working with Microsoft Office (Microsoft Word, PowerPoint, Excel, One Note, and Outlook).
  • Knowledge of:
    • Children's Therapeutic Support Services.
    • County and community resources for mental health, substance abuse disorder, social services, and basic needs.
    • Social work best practices and ethics.
    • Issues related to equity and ability to demonstrate a commitment to promote diversity and inclusion.
  • Strong interpersonal, organizational, computer, and person-centered interviewing skills.
