Our consumers are located primarily in the six-county metro areas. A successful candidate enjoys variety from day to day and is comfortable managing a diverse array of clients. Case Managers have independence while managing their caseloads; someone who prefers group work to be self-led, consulting and collaborating with others, then returning to independent tasks, would succeed in this role.
- Case Managers work with persons served on CADI, DD, BI and/or AC/EW waivers. These persons served may have a developmental disability, serious and persistent mental illness, chemical dependency issues, a physical disability, or be aging.
- Assess the areas of the persons served life where assistance is needed.
- Offer and coordinate resources for the persons served being supported.
- Monitor service delivery and work with providers to ensure the individual’s needs are met.
- Develop service plans.
- Attend regular in-person meetings with persons served and their interdisciplinary teams.
- Maintain and create accurate Case Notes and Billable Units.
- Stay up to date with case management systems, software, and DHS and County resources.
Non-LSW wage: $49,000/year salaried for 40hrs/week.
LSW wage: $51,600/year salaried for 40hrs/week.

Work Schedule:
Employees may qualify for a hybrid schedule (3 remote days and 2 in-office days per week) after completing required training, meeting performance and billable unit expectations - MSSA membership & paid CEUs

Required Qualifications
- Be a graduate from an accredited four-year college with a major in social work, psychology, sociology, or a closely related field
or
- Be a graduate from an accredited four-year college with a major in any field
and
- One year of full-time experience as a social worker/case manager/care coordinator in a public or private social service agency.
or
- One year of experience in education or treatment of persons served with developmental disabilities or related conditions
or
- At least one course focused on developmental disabilities is required. Candidates must provide an official college or university transcript to verify completion of this coursework.
- Applicants must have a valid driver’s license and acceptable driving record as determined by Pinnacle Services policy. Applicants must have a vehicle that is covered with current insurance.
- Applicants must successfully clear local and/or stated background checks.
- Possess a LSW or higher form of BOSW Licensure.
