Provide hands-on daily living support to individuals with disabilities in their homes and communities. Build real relationships and make a lasting difference.

Requirements

• Valid MN driver s license
• Reliable transportation
• Compassionate, patient, and dependable
• Experience preferred but not required

Full-Time Minneapolis / St. Paul

Help individuals with disabilities find, prepare for, and maintain meaningful employment. Coach, advocate, and celebrate wins alongside your clients.

Requirements

• Experience in job coaching or vocational rehab preferred
• Knowledge of MN employment resources a plus
• Valid MN driver s license

Guide individuals transitioning out of facility care and into community living. Coordinate housing, benefits, and support services from start to finish.

Requirements

• Experience in case management or social services
• Familiarity with MN waiver programs preferred
• Strong organizational skills
• Valid MN driver s license

Part-Time / On-Call Twin Cities Metro

Provide short-term relief care for families and caregivers. Flexible scheduling — great for students, retirees, or anyone looking for meaningful part-time work.

Requirements

• Caring and reliable
• Ability to pass background check
• CPR/First Aid certification (or willingness to obtain)
• Experience with disabilities a plus
