Are you detail-oriented, organized, and passionate about making a meaningful difference in the lives of older adults? Everbright Legacy Law PLLC, a mission-driven elder law firm, is seeking a compassionate and experienced Elder Care Coordinator.
Example of Duties:
In this role, you will serve as a vital bridge between the legal, healthcare, and personal care needs of our clients. You will support aging clients and their families in navigating long-term care decisions, ensuring quality of life while maintaining dignity and autonomy. The Elder Care Coordinator collaborates with attorneys and case managers to develop comprehensive Life Care Plans and offers hands‑on client support throughout the aging process.
The following are examples of major job duties expected for this position to perform.
Attend initial office conference with the Attorney, client, and client’s family.
Conduct onsite comprehensive assessments addressing physical, mental, and environmental factors.
Assist in the development and implementation of individualized Life Care Plans.
Coordinate care services and referrals with clients, their families, and healthcare professionals.
Assist in discharge planning from hospitals, Gero psych units, and transitional care units.
Work with families to attain appropriate placement at long‑term care facilities.
Attend Care Conferences as needed.
Assist clients and families in locating assisted living communities and memory care units.
Provide emotional support and education to clients and their families.
Serve as a point of contact and advocate during care transitions or crises.
Maintain detailed documentation of visits, assessments, and care updates.
Build strong working relationships with local long-term care providers and community resources.
Represent the firm professionally through outreach, networking, and occasional speaking engagements.
Qualifications
Below are the qualifications that will enable success in the role.
Licensed LSW, with ability to maintain licensure in good standing.
5 years of experience in geriatric case management, care coordination, or elder-focused social work.
Experience in client discharge and placement.
Strong knowledge of senior housing, long-term care systems, and aging-related resources.
Knowledge and experience working with Medicare, Medicaid, and Waiver programs.
Excellent verbal and written communication skills.
Ability to work independently and manage multiple priorities in a dynamic, fast‑paced setting.
Proficient in Microsoft Office (Outlook, Word, Excel) and other office technology.
Valid driver’s license, clean driving record, and access to a fully insured vehicle for frequent local travel.
