IOCP is searching for its next Case Manager with a focus on IOCP’s Permanent Supportive Housing Program. Permanent Supportive Housing supports individuals and families to obtain and maintain their housing successfully through shelter exit, coordinated entry and through stability. - Case Management including: building ongoing relationships with clients, supporting clients in reaching financial stability through financial review and budgeting, helping clients navigate crises, and empowering clients to create and follow plans to reach personal goals and improve stability. This role will connect clients to external resources, offer budgeting and financial assistance, and provide information and referrals, advocacy, counseling, and problem-solving support.
- Permanent Supportive Housing program administration. Assess immediate health, safety and housing stability needs upon move-in. Provide crisis intervention services to help clients stabilize their housing as a foundation for further work. Help clients create and follow individualized plans focuses on achieving stability goals, including addressing factors that have contributed to past homelessness. Help clients build crisis-management and problem-solving skills. Develop and maintain a collaborative, accountable working relationship with housing management staff through regular communication, role clarity, and joint problem-solving that supports resident concerns.
Conduct monthly budget and assess monthly expenses with Housing Support clients. - Support clients by helping to identify and navigate their barriers to stable housing.
- Client Engagement including internal and external outreach efforts to identify potential residents. Manage the recruitment and screening process including collaboration with Hennepin County shelter staff. Welcome clients, provide orientation to services and complete required intake paperwork and processes. Assist clients to establish their new household and address basic needs. Help introduce clients to IOCP programs and invite their participation.
- Administer rent assistance programs. Assess clients’ need for short-term rent assistance as a tool for increasing long-term stability. Determine eligibility and collect required documentation. Work with clients to make rent assistance plans and monitor progress. Facilitate rent payment requests and engage with property managers.
