POSITION SUMMARY
LSW Social Worker
POSITION DUTIESProvides services to customers and their families/significant others to help them adjust to illness, injury, or hospitalization. Assists the health team in understanding the significance of social, economic, emotional, and family factors in relation to the customer's illness, treatment, recovery, or dying processes. Facilitates timely, efficient, and appropriate discharge of customers and coordinates with next level of care, including transmission of an appropriate plan of care.
Identifies needed community services and coordinates appropriate services for customers in need.
- LSW in the State of Minnesota
- 2+ Years Hospital Social Working Experience
- Discharge Planning Experience
- All Patient Populations Experience
- Epic Experience
10 weeks
SHIFT / HOURS PER WEEK8:00 AM - 5:00 PM, with every 7th weekend.

SALARY$1600.00 - $2400.00
