Overview
Pinnacle Services is hiring a Case Manager for Ramsey County. The role supports people with diverse needs in a fast-paced, collaborative environment that emphasizes person-centered thinking and employee growth. The Case Manager will work with clients primarily in the six-county metro area, with independence in caseload management and opportunities for consultation and collaboration as needed.
Job Responsibilities and Duties- Assess areas of the person served life where assistance is needed.
- Offer and coordinate resources for the person served being supported.
- Monitor service delivery and work with providers to ensure the person’s needs are met.
- Develop service plans
- Attend regular meetings with the person served and their interdisciplinary teams.
- Document case notes to create billable units.
Non-LSW wage: $49,000/year, salaried for 40 hours/week.
LSW wage: $51,600/year, salaried for 40 hours/week.

Work Schedule:
3 remote days and 2 in-office days per week after completing required training and performance expectations - MSSA membership & paid CEUs

Required Qualifications
- Be a graduate from an accredited four-year college with a major in social work, psychology, sociology, or a closely related field
or
- Be a graduate from an accredited four-year college with a major in any field
- One year of full-time experience as a social worker/case manager/care coordinator in a public or private social service agency
or
- One year of experience in education or treatment of persons served with developmental disabilities or related conditions
or
- At least one course focused on developmental disabilities is required. Candidates must provide an official college or university transcript to verify completion of this coursework.
- Applicants must have a valid driver’s license and acceptable driving record as determined by Pinnacle Services policy. Applicants must have a vehicle that is covered with current insurance.
- Applicants must successfully clear local and/or state background checks.
- Possess a LSW or higher form of BOSW Licensure.
