The Housing Stability area of Human Services Public Health is seeking two Case Management Assistants to join the Homelessness Prevention team to support residents remaining in or newly accessing stable housing and avoid homelessness. These positions will support the Homelessness Prevention team's eviction prevention strategy, and will provide triage services at housing court, respond to calls from residents needing assistance to maintain their housing, provide navigation and support to residents with housing instability, and support continuous improvement projects with the team and Hennepin County.

Location and hours:
These positions are hybrid and will be performed both on-site at the East Lake Service Center, 2215 Lake Street East, Minneapolis, MN 55407, and remote as job duties require. Work hours will be Monday through Friday, 8-hour shifts between the hours of 7 a.m.

- 6 p.m. While these positions are designated as hybrid, based on current requirements hires must reside in or relocate to Minnesota or Wisconsin. New Employee Orientation (NEO) for all new benefit-earning employees is in-person in downtown Minneapolis. Details about NEO will be provided upon hire.

About these position types:
These are limited duration (temporary) full-time, benefit earning position expected to last up to two years. These positions are internally classified as Case Management Assistants.  to view the job classification specification.

In these positions, you will:

• Provide triage and navigation for residents at risk for eviction, including resource referrals for legal representation, benefits, or supportive services.
• Assist residents with initiating or completing applications for emergency rent assistance.
• Maintain respectful and trauma-informed customer service practices.
• Follow Rent Help Hennepin eligibility policies, keeping up to date on policy and procedure changes.
• Complete triage forms and refer residents to appropriate resources.
• Communicate in a professional and timely manner with customers and housing partners.
• Respond to application inquiries from resident and property managers.
• Follow team communication guidelines when working with community partner agencies.
• Attend all required one on one, unit, and program meetings.
• Contribute to a positive, inclusive work environment supporting team members, colleagues, and customers.
• Complete other special projects, as assigned.

Need to have:

• One of the following:
• Two years or more of responsible, paid social service experience providing direct client service.
• A two-year human services certificate.
• Two years or more of college coursework in the behavioral sciences.

Nice to have:

• Experience:
• Providing triage and navigation for residents at risk of losing their housing.
• Assisting residents in initiating or completing applications for rent assistance.
• Using the Rent Help Hennepin system.
• Working with community partners serving residents at risk of eviction and homelessness.
• Using Microsoft Office Suite (Excel, Outlook, Teams, and/or Word).
• Knowledge of community resources and social services available to clients.
• Ability to:
• Work effectively and communicate with individuals from a variety of cultural and lifestyle backgrounds in a respectful and non-judgmental manner.
• Make sound, independent judgment.
• Work effectively and collaboratively with external and internal partners.
