Job Overview

Ebenezer is looking for a Resident Services Specialist to join our Senior HUD team at Tower Apartments in Minneapolis, MN! The Resident Services Specialist assists residents living in HUD independent living to obtain needed supportive services from the community, to enable them to continue living as independently as possible and age in place in their homes.

This resident services schedule includes;

* 80 hours every two weeks

* Full time;
Day shift

* No weekends

Responsibilities:

* Provide case management:

* Intake, referral services, evaluation of health, psychological and social needs.

* Development of individually tailored case plan for services and periodic re-assessment of the resident's situation when services are not available through the general community.

* Contact resident when incidents occur with staff, their family members, or neighbors to provide supportive counseling.

* Educate residents on available services, application procedures, client rights, providing advocacy as necessary. Refer residents to the appropriate agency.

* Develop and monitor ongoing provision of services from community agencies and update case management and provider agency with the progress of the resident.

* May attend care conferences (if applicable) to advocate services are in place prior to resident returning home from an extended care facility.

* Complete and submit, timely and accurately, all records, reports, and documents required by the property and federal and/or state agencies:

* Keep and maintain resident services resident files.

* Proper documenting in the American Association of Service Coordinators (AASC) software for annual and semi-annual HUD reporting.

* Provide information and resources to be used by property staff, residents, and their family members:

* Organize support programs with community organizations.

* Assist residents in building an informal support network, coordinating and/or providing training to residents in the obligations of tenancy, and educating Company staff on issues relating to the aging population.

* Establish contacts with agencies and service providers in the community:

* Assist with welcoming new residents and providing orientation; assess needs they may have and connect with agencies and services as needed.

* Schedule service providers in to discuss necessary aging in place topics of value to residents and their family.

* Oversee and manage all meal and food support programs through partnership with local organizations (prepared meal programs, NAPS, food bank, CES).

* Drives Ebenezer van/bus for resident outings:

* Reviews and completes Vehicle Requirements - Transporting Residents Checklist prior to each resident outing.

* Maintains a log of transportation services and tasks performed throughout the driving period.

* Responsible for the safety and welfare of passengers.

* Practices sound, safe judgment when performing transportation services including but not limited to fire/disaster procedures and infection control procedures.

* Recognizes and acts appropriately on safety hazards and incidents.

* Follows all laws with regard to operating a motor vehicle.

* Immediately reports any traffic violations or accidents.

Required Qualifications:

* Vocational School or
- * Training or Associate Degree

* 3 years of previous experience

* Annual HUD Training (obtain 12 hours of training annually)

* Drivers License

* Medical Exam Certification

* Must be at least 18-years old

* Must be insurable under the employers Auto Insurance Coverage

* Must complete and pass in-house transportation curriculum course and pass driving evaluation. within 30 Days

Preferred Qualifications:

* Bachelor's Degree:
List - Social Work, Gerontology, Psychology or Counseling

* 5 years of previous experience

* Minnesota Social Worker License

* AASC Certification
