Family Rapid ReHousing Intake CoordinatorPosition Overview:
Agate's Housing Programs Department serves households who are experiencing or at risk of homelessness in Hennepin County. Our programs include Prevention, Diversion, Coordinated Entry Assessments, Fast Track, Rapid Rehousing, and Long-term Supportive Housing for households with children and adults without children. By offering programs of various intensity and length, we hope to be able to provide the level of service each household needs to be successful.
The goal in all of our housing programs is to end homelessness by assisting households in stabilizing their housing and achieving self-sufficiency.
The objective of the Family Rapid ReHousing Intake Coordinator is to be the primary contact with the Coordinated Entry System in Hennepin County. They will complete intakes with families and individuals referred to Agate's Rapid ReHousing program. They will determine eligibility and connect new referrals to assigned case managers so they can assist families in obtaining permanent housing. This role is responsible for developing and maintaining strong, collaborative relationships with case managers, program managers, and housing locators.
This position currently supports the Minneapolis Crisis funds within Prevention services, and other time limited special projects.
- Work with Coordinated Entry in Hennepin County to receive referrals for families and individuals experiencing homelessness.
- Complete intake appointment with families referred to Rapid ReHousing by Coordinated Entry.
- Determine eligibility for families and accept/decline referrals through Coordinated Entry System.
- Enroll client in Family Rapid ReHousing program and upload all intake paperwork into internal database and complete HMIS entry paperwork.
- Coordinate with Rapid ReHousing Managers to connect client with case managers.
- Educate clients about tenant responsibilities and rights, financial management, and affordable housing opportunities.
- Work cooperatively and collaboratively with other agency staff and Hennepin County shelter staff to meet the clients' needs.
- Assist with Prevention applications as needed. This position currently supports the Minneapolis Crisis Funds and any other time limited special projects.
- Manage multiple fund sources to assist clients and maintain accurate records of expenditures.
- Complete accurate case notes, data collection, and documentation for assigned clients in a timely manner.
- Work within the community to complete assessments and attend community events. This may include buildings that are only accessible by climbing stairs.
- Performs all other duties, as assigned.
