Summary
Native American Community Clinic opened its doors in 2003 to address the health disparities within the urban Native American community of the Twin Cities. Our mission is to promote the health & wellness of mind, body, and spirit of Native American families. We offer a full range of healthcare services that include Medical, Behavioral Health, Dental, and Substance Abuse programs. NACC approaches healthcare by addressing root causes of health disparities including access to food, housing and health insurance with services such as resource navigation, care coordination, outreach and community-based activities through the use of our peer recovery coaches and community health workers.
NACC strives to honor health and tradition by providing spiritual care and access to traditional healing through our Elders in Residence.
The Native American Community Clinic (NACC) is a community health center in Minneapolis dedicated to serving urban Indigenous communities and other underserved groups. We are seeking a compassionate, resourceful, community-oriented Resident Connections Coordinator to join our team. This position plays a vital role in supporting residents with social service needs, including assistance with maintaining safe/stable housing, resources/referrals for children 0-18 yrs of age, health insurance, food security, transportation information, and other essential supports.
The Resident Connection Coordinator works closely with internal/ external teams and community partners to remove barriers to care, improve access to resources, and enhance overall patient well-being through culturally responsive, holistic support.
Wihinaphe Apartments, a new affordable housing community located above the new Native American Community Clinic in Minneapolis, opening in September 2026. This housing development is being led by Wellington Management in partnership with Native American Community Clinic (NACC) and will include:
- 83 affordable apartment units
- Studio, 1, 2, 3, and 4-bedroom options
This position is part of a Supportive Housing Services partnership between NACC and Simpson Housing Services, the position will provide supportive services to residents at the Whinaphe Apartment Building for people with disabilities that provides formerly homeless single adults and families with a safe and permanent home and support services to help residents regain and maintain stability in their lives, homes and the community.
This energetic and adaptable employee will work as part of a team to assist residents to maintain housing, support access to community resources, and foster community engagement and resident leadership. The Resident Connections Coordinator (RCC) must be knowledgeable about and able to respond effectively to the complexities of participants’ lives and the co-occurring challenges and conditions associated with long-term homelessness.
The RCC will work as part of a team comprised of staff from different agency partners (Simpson, Wellington). Hours for this position are Mondays to Fridays, 9-5pm.
Responsibilities- Welcome new residents and actively build relationships, explaining the resident services program, and offering information and support to those wishing to access resources.
