Board Certified/Eligible Primary Care Physician

Place of Performance:
The Contractor shall furnish services at the SCVAHCS, Montevideo Community Based Outpatient Clinic located at: 814 N. 11th St, Montevideo, MN 56265. Saint Cloud Health Care System Hours of Performance:
The Contractor will provide Primary Care services from 08:00 a.m. – 4:30 p.m., Monday through Friday and any other mutually agreed upon hours.

Care coverage will be provided at 80 hours per week plus up to five (5) hours weekly beyond normal tour of duty for approved activity (i.e. training, patient, care activities, and administrative duties). Patient Care appointments are scheduled. The average workload is an average of 12 patients a day varying between 30 minutes and 60 minutes appointment times. Assessment of walk-in outpatient veterans varies in workload but is required in the PACT responsibility of duties.

Qualifications:

Resume must include the following qualifications and supporting documentation:

  • License:
    The Physician(s) (MD/DO) proposed and accepted by the Government to perform services under this contract shall hold a current, full, and unrestricted license to practice medicine in any State, Territory, Commonwealth of the United States, or the District of Columbia when services are performed onsite on VA property.
  • Board Certification:
    All provider(s) shall be Board Certified by the American Board of Internal Medicine or the American Board of Family Medicine being accepted as an additional certification.
  • Currently certified in Basic Life Support (BLS) and Advanced Cardiac Life Support by the American Heart Association (AHA), (A copy of the front and back of an American Heart Association BLS course card shall be provided).
  • Degree Proof
  • DEA Registration:
    Copy of current DEA certificate of registration for the state in which services will be provided.
  • National Provider Identifier (NPI)
  • OIG Exclusion Check
  • Malpractice and Medical Liability Insurance Certificates

Job Duties and Responsibilities:

Direct Patient Care:
The VA anticipates that the contracted provider(s) serving under this contract will spend an estimated 80% of their time involved in direct patient care. Scope of Care:
The Contractor's Physician(s), Advanced Practice Nurse Practitioner(s) or Physician's Assistant(s) (as appropriate and within scope of practice/privileging) shall be responsible for providing Primary Medicine Care promoted in a team-based, patient-centered care focusing on personalized, integrated, comprehensive, and coordinated approach to health care, including, but not limited to:
Preventative Care, Acute Care, Chronic Disease Management, Health and Wellness, Diagnosis, Evaluation, Treatment, Care Management, and Consultation.
