We're seeking a dedicated and compassionate Physical Therapist (PT) for a contract position in a Skilled Nursing Facility (SNF) setting. This role is based in Wabasso, Minnesota
, with scheduled floating to Franklin, MN two days each week.

This is a great opportunity for a motivated Travel PT or Contract Physical Therapist who enjoys working in a collaborative environment, focusing on quality care and patient progress in a supportive SNF setting.

• Evaluate, plan, and implement physical therapy treatment programs for patients in SNF and sub-acute rehab settings.
• Create individualized therapy plans to improve strength, balance, and mobility.
• Accurately document treatments, progress, and outcomes in accordance with facility and payer requirements.
• Collaborate with OTs, SLPs, nursing, and interdisciplinary team members to deliver patient-centered care.
• Maintain an 85% productivity standard.
• Provide therapy coverage at both Wabasso and Franklin facilities as scheduled.

• Current and valid Physical Therapist (PT) license in the state of Minnesota (or eligibility for MN licensure).
• Graduate of an accredited Physical Therapy program.
• Previous SNF or geriatric rehab experience preferred.
• Excellent communication, documentation, and organizational skills.
• Reliable transportation for travel between sites.
