Job Summary:

To provide direct resident care services according to the plan of care prepared by and under direct supervision of the Charge Nurse.

Work Environment:

Works in well lighted well-ventilated work area.

Essential Functions:
  • Provide direct resident care services with attention directed toward improving or maintaining optimal physical, mental, and psychosocial functional performance, promoting resident choice and preferences, and increasing resident dignity and quality of life according to each resident’s plan of care and the NA Standards of Care.
    • Ask resident their favorite food and beverage choices.
    • Serve resident meal and snacks with correct diet and consistent in a timely manner to ensure proper temperature.
    • Assist, cue, or monitor residents with eating.
    • Encourage adequate intake as directed.
    • Record intake as directed.
    • Supply fresh water to residents during each shift.
    • Measure weight for resident as directed.
    • Watch for and report to nurse changes in resident appetite, intake, and ability to eat.
  • Continence care, including:
    • Support use of bedpan, urinal, commode, or toilet as appropriate.
    • Follow established bowel and bladder program.
    • Collect routine urine, stool and sputum samples as directed.
    • Keep urinary catheter and tubing properly positioned and cleaned.
    • Empty and record output for each shift as directed.
    • Provide colostomy care as directed, observe for results as directed.
    • Record bowel movements and character as directed.
    • Keep incontinent residents dry and clean.
    • Cleanse after each incontinent voiding or bowel movement.
    • Watch for and report to Charge Nurse changes in resident elimination patterns.
  • Mobility assistance, including:
    • Assist with ambulation to the extent needed.
    • Use walker, cane, wheelchair, gait belt and other mechanical aids according to the care plan.
    • Transfer resident between bed, wheelchair, geriatric chair, or commode using assistive devices, gait belt and proper body mechanics.
    • Maintain optimum joint functioning for activities of daily living by positioning using proper body alignment.
    • Move and regularly reposition residents who are lying in bed.
    • Use proper body mechanics and turning techniques.
    • Encourage residents to be up and as active as possible.
    • Conduct range of motion exercises as directed.
    • Assist with rehabilitation and restorative programs and goals according to each resident’s care plan.
    • Encourage resident participation in therapeutic activities.
    • Assist residents to and from social and therapeutic activities.
    • Watch for and report to Charge Nurse changes in resident mobility patterns or safety.
    • Report incidents or injuries to the Charge Nurse immediately.
  • Personal hygiene cares, including:
    • Assist with bathing‑bed, whirlpool, or shower.
    • Assist with shampooing and hair care.
    • Assist with skin care needs.
    • Assist with grooming and perineal care.
    • Assist with dressing and undressing.
    • Assist with oral and denture hygiene.
    • Assist with nail care.
    • Assist with shaving.
    • Watch for and report to nurse changes in resident ADL abilities or skin integrity.
  • Comfort and quality of life needs, including:
    • With attention directed toward embracing the Culture Change philosophy, make decisions and carry out duties according to the resident’s preferred routines and choices.
    • Assist the resident in meeting the needs for uninterrupted rest and naps.
    • Watch for signs of discomfort and report to the nurse immediately.
    • Report resident complaints of discomfort to the nurse immediately.
    • Follow the resident’s care plan choices.
    • Assist with socialization, exercise, and mental stimulation of the resident.
    • Maintain a clean, comfortable, and pleasant environment.
    • Change bed linens and make beds.
    • Tidy the resident’s room.
    • Observe proper ventilation, temperature, light, and noise control.
  • Effective communication with residents, families, and other staff by:
    • Communicate therapeutically and respectfully with residents by using terminology, manner of speech and non‑verbal techniques appropriate to the individual’s level of understanding.
