Job Summary

The Home Health Aide is responsible for assisting clients with tasks of daily living as designated by the assignment sheet established by a registered nurse. The Home Health Aide will be supervised by a registered nurse and/or Nursing Supervisor. May also assist with Public Health Nurse Clinic providing foot care.

Responsibilities
  • Performs bathing, bedding, hair care, and other personal care as directed, under the supervision of the nurse.
  • Performs range of motion exercises, assists with ambulation and transfers with the use of equipment or assisting devices i.e. walkers, wheel chairs, hospital beds, etc., under the supervision of a Registered Nurse and/or physical therapist.
  • Assists the patient with activities of daily living including personal care, meal preparation, and light housekeeping activities.
  • Offers emotional support and promotes the patient’s alertness through conversation, reading, etc.
  • Performs clean dressing changes under the supervision of a Registered Nurse.
  • Encourages patient and/or family to become as independent as possible within medically determined limitations.
  • Provide reminders to the patient for medication, under the direction of a Registered Nurse.
  • Reports any changes and/or concerns in the patient’s mental or physical condition to the Registered Nurse.
  • Requests the family to call the physician in the case of an emergency and if the family is not available the Home Health Aide must call the nurse for instructions.
  • Completes reports and records services rendered while in the home.
  • Maintains a healthy, safe environment and/or as a part of an interim plan to maintain the client unit in the home.
  • Maintain client confidentiality and be compliant with the Health Insurance Portability and Accountability Act (HIPAA).
  • Miscellaneous
  • Participates in mandatory in-service programs for Home Health Aides and staff meetings.
  • Attends classes, seminars, etc., to maintain certification.
  • Maintains CPR certification.
  • Attends case conferences for team staffing regarding clients they service.
  • Assist in the response to Public Health emergencies as directed by the CHS Administrator/Public Health Director.
  • Performs other related duties as directed.
