Home Health Occupational Therapist (OTR/L) – Functional Living Specialist
Job Title: Occupational Therapist (OTR/L) | Build Independence, One Home at a Time
Schedule: Full-Time | Monday–Friday
Compensation: $80,000–$150,000 annually

- Evaluate patients in their homes and develop individualized occupational therapy treatment plans.
- Improve independence with activities of daily living, home safety, upper extremity function, cognition, and adaptive techniques.
- Recommend home modifications and adaptive equipment that support long-term independence.
- Educate patients and caregivers on strategies that improve safety and function.
- Collaborate closely with physicians and the interdisciplinary care team to achieve outstanding patient outcomes.
- Active Minnesota Occupational Therapist (OTR/L) license.
- Strong clinical reasoning and patient-centered care philosophy.
- Excellent communication and organizational skills.
- Home health experience is helpful but not required.
- Valid driver's license and reliable transportation.
