Direct Support Professional – $500 Sign‑On Bonus – Crookston

Hourly Rate: $14.00 / hr
Shifts Needed:
Full Time or Part Time – Monday to Friday 3:00 PM to 11:00 PM, Saturday and Sunday AM’s and PM’s

Essential Functions

Direct Services: Provide direct services to residents by implementing individualized support plans created by the resident’s care team.

Program Planning: Assist in implementation of program plans, communicate with DC/ADC, and offer constructive feedback during staff meetings.

Documentation: Document, participate in, and follow through with all programming. Accurate records demonstrate progress toward each resident’s outcomes.

Daily Progress: Gather and record resident progress daily to demonstrate measurable outcomes.

Areas of Support: DSPs provide training, counseling, and assistance in the following areas:

  • Personal hygiene and grooming, including incontinence support
  • Care of clothing and personal belongings
  • Understanding and exercising rights in the community
  • Fulfilling responsibilities in the community
  • Personal awareness
  • Routine health care
  • Appropriate socialization
  • Community orientation
  • Recreation and leisure activities
  • Meal preparation and planning
  • Dietary oversight
  • Housekeeping and home maintenance
  • Developing interpersonal relationships
  • Emergency procedures
  • Personal program participation
  • Anger management
  • Money management
  • Caring for personal property
  • Behavioral intervention
  • Addressing medical needs

Resident Routines: Follow through with each resident’s established routines.

Resident Rights: Ensure resident rights are protected and observed.

Training and Meetings: Attend all mandatory staff meetings and scheduled trainings.

Environment: Create and maintain a homelike, nurturing environment respecting the resident’s home.

Health, Safety, and Welfare: Protect the health, safety, and overall well‑being of each resident.

Policies and Procedures: Follow all training‑outlined policies and procedures.

Reporting Structure: Report to the Designated or Assistant Designated Coordinator and address concerns respectfully.

Physical Environment: Maintain a clean, safe, and orderly home environment.

Household Chores: Follow assigned chore lists to assist in keeping the home clean.

Teamwork and Professionalism: Work collaboratively, treating all individuals with respect and professionalism.

Attendance and Punctuality: Prioritize resident needs while accommodating staff schedules when possible.

Confidentiality: Maintain confidentiality of home and resident matters; direct concerns only to the DC.

Other: Perform additional duties as assigned.

Education and Experience

High School Diploma or GED required.

Must obtain and maintain active First Aid and CPR certification; must complete crisis de‑escalation or therapeutic intervention training when scheduled.

Physical Requirements
  • Work in a program location with occasional outdoor community activities.
  • Frequent sitting, grasping, feeling, talking, hearing, and repetitive motions.
  • Occasionally lift up to 50 lbs and perform bending, twisting, stooping, squatting, pushing, pulling, standing, crawling, reaching, walking, driving, traveling, kneeling, and crouching.
