Job Title: Direct Support Professional - $500 Sign On Bonus - Crookston (MN)
Hourly Rate: $14.00 / hr
Shifts: Full Time or Part Time – Monday to Friday 3:00 PM to 11:00 PM;
Saturday and Sunday AM or PM
Work Location: Crookston, MN

- Direct Services – Provide care per the support plan created by the resident’s team (Direct Care Coordinator, Case Manager, Guardian, Day Program staff)
- Program Planning – Assist in implementing individual program plans and provide constructive feedback during staff meetings
- Documentation – Maintain accurate records, participate in, and follow through with all programming
- Daily Progress – Gather and record information about resident progress each day to demonstrate measurable outcomes
- Resident Routines – Follow through with each resident’s established routines
- Resident Rights – Ensure resident rights are protected and observed
- Training and Meetings – Attend all mandatory staff meetings and scheduled trainings
- Environment – Create and maintain a homelike, nurturing environment respectful of the home setting
- Health, Safety, and Welfare – Protect health, safety, and overall well‑being of each resident
- Policies and Procedures – Follow all policies and procedures as outlined in training
- Reporting Structure – Report to the Designated or Assistant Designated Coordinator; bring concerns to the DC respectfully
- Physical Environment – Maintain a clean, safe, and orderly home environment
- Household Chores – Follow any assigned chore list to keep the home clean
- Teamwork and Professionalism – Work together as a team; treat all individuals with respect and professionalism
- Attendance and Punctuality – Schedule serves residents’ needs first; accommodate staff schedules when possible
- Confidentiality – Protect confidentiality of all home matters; share only with the DC as needed
- Other – Perform additional duties as assigned

Education: High School Diploma or GED
Experience: Must obtain and maintain active First Aid and CPR certification; must obtain crisis de‑escalation or therapeutic intervention training when scheduled
Physical RequirementsWork may be in a program location with occasional outdoor community work; frequent sitting, grasping, conversing, hearing, and repetitive motions; occasional lifting of 50 lbs; occasional bending, twisting, stooping, squatting, pushing, pulling, standing, crawling, reaching, walking, driving, traveling, kneeling, and crouching
Knowledge, Skills & AbilitiesKnowledge: Strong understanding of state and federal regulations, person‑centered philosophy, client service needs, medication administration protocols, emergency and disaster response procedures, mandated reporting requirements, and positive behavior support practices
