MDS Coordinator - Cura of Le Sueur

Duties

• Coordinate and oversee the MDS (Minimum Data Set) process for patients in a healthcare setting
• Ensure accurate and timely completion of MDS assessments and documentation
• Collaborate with interdisciplinary team members to gather necessary information for MDS assessments
• Review medical records and interview patients, families, and caregivers to gather relevant information
• Analyze data and identify potential issues or discrepancies in MDS assessments
• Develop care plans based on MDS assessments and collaborate with healthcare professionals to implement appropriate interventions
• Monitor and evaluate patient progress and update care plans as needed
• Provide education and training to staff on MDS documentation requirements and processes

• Proficiency in using point click care or similar electronic health record (EHR) systems
• Experience working with children, including knowledge of child development and age-appropriate care practices
• Knowledge of infection control protocols and practices to ensure patient safety
• Familiarity with family planning services and resources
• Knowledge of discharge planning processes and resources for smooth transitions of care
