Provide direct in-home physical therapy under the direction of a licensed Physical Therapist;
Instruct patients in exercises and the use of mobility aids;
Maintain clinical records and notes within 24 hours of care;
Collaborate with Home Health team and families to optimize patient outcomes;
Observe patient responses to treatment and report changes to supervising PT
