Occupational Therapist – Rehabilitation Services, Wheaton, Minnesota
A growing healthcare rehabilitation organization is seeking a compassionate and motivated Occupational Therapist to join its multidisciplinary therapy team. About the Role- Provide occupational therapy services to patients across multiple care settings
- Evaluate patient needs and develop individualized treatment plans
- Collaborate with physicians, therapists, nursing staff, and caregivers to achieve optimal outcomes
- Deliver evidence-based interventions focused on improving functional independence
- Monitor patient progress and modify treatment plans as appropriate
- Educate patients and families regarding treatment goals and home programs
- Conduct occupational therapy evaluations and assessments
- Develop and implement individualized treatment plans
- Provide direct patient care in outpatient, inpatient, and community-based settings
- Maintain accurate and timely clinical documentation
- Participate in interdisciplinary care planning and team meetings
- Educate patients, families, and caregivers on therapeutic strategies
- Ensure compliance with professional standards and regulatory requirements
- Support quality improvement initiatives and best practices
- Graduate of an accredited Occupational Therapy program
- Active Occupational Therapy license or eligibility for licensure in Minnesota
- Strong clinical, communication, and interpersonal skills
- Ability to work independently and collaboratively within a multidisciplinary team
- Commitment to delivering exceptional patient-centered care
- Previous rehabilitation experience preferred but not required
