OccupationalTherapist (OT) | Home Health | Position Details
Provides skilled occupational therapy services to patients in their homes to restore function, prevent disability, and promote rehabilitation. Evaluates patients, develops individualized treatment plans, and delivers evidence-based therapeutic interventions that support independence, safety, and quality of life. Collaborates with an interdisciplinary care team to provide compassionate, patient-centered home health services.
Key Responsibilities- Evaluate patients to determine functional limitations, rehabilitative needs, and appropriate occupational therapy interventions.
- Develop, implement, and update individualized treatment plans in collaboration with the interdisciplinary care team and according to home health policies.
- Provide skilled occupational therapy services in the home and community setting to improve functional performance, independence, and safety.
- Monitor, evaluate, and document patient progress, modifying treatment plans as clinically appropriate.
- Ensure physician or authorized practitioner orders are obtained prior to initiating home health therapy services.
- Collaborate and communicate with the RN Case Manager, Therapy Lead, physicians, and other healthcare professionals to coordinate patient care.
- Accurately document evaluations, treatments, patient responses, and progress in the electronic medical record in accordance with regulatory and organizational requirements.
- Educate patients, families, caregivers, and other support persons on rehabilitation techniques, home exercise programs, adaptive equipment, safety strategies, and disease management.
- Recommend, order, and maintain adaptive equipment and assistive devices as appropriate to support patient independence.
- Provide in-service education and clinical support to department staff regarding rehabilitation services and patient care needs as requested.
- Participate in department meetings, required education, and ongoing professional development to maintain current knowledge of occupational therapy practice and home health regulations.
- Promote positive patient and family experiences through compassionate, respectful, and professional care.
- Maintain patient confidentiality and comply with all applicable regulatory standards, accreditation requirements, and organizational policies.
- Support departmental goals, quality improvement initiatives, and community relations through excellence in patient care.
- Perform other duties as assigned.
- Graduate of an occupational therapy program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) or its successor organization.
- Current licensure or eligibility for licensure as an Occupational Therapist in the State of Minnesota.
- Current certification through the National Board for Certification in Occupational Therapy (NBCOT), when applicable according to licensure requirements.
- Current registration with the American Occupational Therapy Association (AOTA) preferred.
- Minimum of one (1) year of clinical experience preferred; home health experience is beneficial.
- Valid Minnesota driver's license, reliable transportation, and proof of current automobile insurance.
- Strong clinical assessment, treatment planning, documentation, communication, and organizational skills.
- Ability to work independently while collaborating effectively within an interdisciplinary healthcare team.
- Maintain professionalism, confidentiality, and excellent customer service.
- Communicate effectively in English, both verbally and in writing.
- Pass a background study through the Minnesota Department of Human Services.
