JOB PROFILE Self-Hire Community Habilitation Qualifications- High School Diploma or GED required
- Work Experience in the field of Developmental Disabilities/OPWDD with children or adults preferred
- Sensitivity to the needs of the individual
- Genuine interest in teaching, and willingness to motivate individuals to learn
- Valid New York State License, current insurance and liability coverage
- Strong command of verbal and written communication (English)
- Directly accountable to the individual primary caregivers, and indirectly accountable to the Program Coordinator Self Direction
- Hawthorne Foundation Inc. is the Employer of Record
- A. Directly responsible for the specific individual assigned
- B. Responsible for entering daily notes based on valued outcomes/goals worked on with the SD Individual
- D. Responsible for completing Monthly Summary Notes
- E. Complete assigned documentation
- F. Prepare for emergencies
- G. Responsible for carrying out daily schedules as developed and planned
- H. Ensures that individual is properly accounted for when transitioning to and from various locations
- I. Implements weekly and daily programs to assist consumer in attaining his/her
- J. Coordinates meaningful community integration opportunities for the specific Individual
- K. Punching in and out via an EVV App on an Android or Apple device
