Location: Minneapolis

Sevita is seeking a compassionate Home Health Caregiver to provide personal care and daily living support to individuals in their homes. In this Healthcare & Medical Services role within the Personal Care & Service department, you will assist with bathing, grooming, mobility, meal preparation, and light housekeeping, while promoting independence and dignity. You'll build trusting relationships, follow individualized care plans, and observe and report changes in condition.

Responsibilities

  • Provide hands-on assistance with activities of daily living (ADLs), including bathing, grooming, dressing, toileting, and mobility
  • Support individuals with meal planning, preparation, feeding (as needed), and following any dietary guidelines
  • Perform light housekeeping tasks such as laundry, dishwashing, and tidying to maintain a safe and clean home environment
  • Provide safe transportation to medical appointments, community outings, shopping, and other errands
  • Follow individualized care plans and behavior support strategies as directed by supervisors or clinical staff
  • Monitor and report changes in physical, emotional, or behavioral condition to appropriate team members
  • Document services delivered, observations, and any incidents accurately and in a timely manner
  • Offer companionship, emotional support, and encouragement to promote independence and quality of life
  • Assist with medication reminders or administration as permitted by policy and state regulations
  • Follow all safety protocols, infection control practices, and emergency procedures
  • Participate in required training, staff meetings, and ongoing professional development
  • Collaborate respectfully with families, guardians, and other care professionals to support consistent, person-centered care
