Location: Le Sueur
Evolve Therapy Services, LLC is seeking a passionate Physical Therapy Assistant for an in-house Skilled Nursing Facility located in Le Center, MN. This PRN position supports the Physical Therapist, conducts screenings, and helps plan and implement therapy interventions for residents.
You will work under supervision to provide individualized and group treatments, monitor progress, and assist with documentation in Net Health EMR.
