Sevita is seeking compassionate Direct Support Staff to empower individuals with diverse abilities in community and home settings. In this role, you'll provide person-centered support with daily living skills, community integration, and social and recreational activities, while promoting independence, safety, and dignity. You'll collaborate with families and care teams, follow individual service plans, and document progress. Responsibilities
- Provide person-centered support to individuals with varying abilities in home and community settings
- Assist with activities of daily living, including personal care, hygiene, and dressing as needed
- Support meal planning and preparation, including following dietary guidelines when required
- Provide medication reminders in accordance with agency policies and individual service plans
- Assist with light housekeeping tasks to maintain a safe, clean living environment
- Support individuals in participating in community activities, appointments, recreation, and social events
- Encourage development of independent living skills, communication, and self-advocacy
- Follow individualized support or service plans and document daily activities, progress, and behavioral observations
- Monitor and promptly report changes in health, behavior, or safety concerns to supervisors or clinical team
- Maintain accurate, timely documentation and incident reports in accordance with regulations and agency standards
- Communicate effectively and respectfully with individuals, families, guardians, and interdisciplinary team members
- Promote dignity, choice, and respect for the rights and preferences of each individual served
- Adhere to all safety, infection control, and emergency procedures, including proper use of lifts or adaptive equipment when applicable
- Participate in required trainings, team meetings, and professional development opportunities offered by Sevita
