Life Skills Trainer

Sevita is hiring a Life Skills Trainer in our Healthcare & Medical Services department. In this role, you'll support individuals with varying abilities in building daily living, social, and community integration skills. Responsibilities include implementing person-centered care plans, teaching self-care and household skills, supporting behavioral goals, and documenting progress. Responsibilities

• Provide person-centered support to individuals with disabilities, brain injuries, or behavioral health needs in home and community settings
• Assist with activities of daily living (ADLs), including personal care, hygiene, dressing, and grooming as needed
• Support individuals with meal planning, meal preparation, and safe eating practices
• Offer medication reminders in accordance with organizational policies and care plans (no independent clinical decision-making)
• Teach and coach life skills such as communication, social interaction, time management, and problem-solving
• Support individuals with money management, shopping, and use of community resources
• Provide transportation or travel support to appointments, programs, work, school, and community activities as assigned
• Implement and reinforce behavioral or therapeutic strategies developed by clinical staff
• Observe, document, and report changes in behavior, mood, health status, or skill progress to supervisors and clinicians
• Maintain a clean, safe, and supportive environment, following health, infection control, and safety procedures
• Use adaptive equipment and transfer techniques safely according to training and care plans
• Participate in team meetings, trainings, and supervision to support coordinated, high-quality care
• Communicate professionally and respectfully with individuals served, families, guardians, and team members
• Respond calmly and appropriately to behavioral or medical incidents, following crisis and emergency protocols

Required Skills

• Basic knowledge of caregiving and direct support practices
• Understanding of activities of daily living (ADLs) and personal care support
• Behavioral support and de-escalation techniques
• Observation and documentation of client behaviors and progress
• Basic computer and electronic health record (EHR) or documentation skills
• Safe patient handling and use of adaptive equipment (lifts, transfer devices)
• Time management and organization in a community-based schedule
• Valid driver's license and safe driving skills (for transporting individuals)
• Clear, compassionate verbal and written communication
• Cultural competence and ability to work with diverse populations
• Problem-solving and sound judgment in day-to-day support situations
