Sevita is seeking a compassionate Direct Care Professional to support individuals of all ages and abilities in community and home-based settings. In this role, you'll assist with daily living activities, implement care plans, promote independence, and provide emotional support. You'll collaborate with an interdisciplinary team to ensure person-centered, high-quality care across behavioral health and rehabilitation services. Responsibilities

  • Provide direct support with activities of daily living (ADLs), including bathing, grooming, dressing, toileting, and feeding
  • Assist with mobility, transfers, and the safe use of adaptive equipment as needed
  • Support individuals in following person centered care plans and behavior support plans
  • Prepare simple, nutritious meals and assist with eating according to dietary needs
  • Accompany individuals to appointments, community activities, and recreational outings
  • Encourage skill-building, social engagement, and independence in home and community settings
  • Monitor and document changes in physical, emotional, or behavioral status and report concerns promptly
  • Follow medication assistance procedures as permitted and directed by nursing or supervisory staff
  • Maintain a clean, safe, and organized living environment, following infection control and safety protocols
  • Complete required documentation and electronic records accurately and in a timely manner
  • Participate in required training, team meetings, and professional development activities
  • Collaborate respectfully with families, guardians, and team members to support individual goals

Required Skills
