Sevita is seeking a compassionate Direct Care Professional to support individuals of all ages and abilities in community and home-based settings. In this role, you'll assist with daily living activities, implement care plans, promote independence, and provide emotional support. You'll collaborate with an interdisciplinary team to ensure person-centered, high-quality care across behavioral health and rehabilitation services. Responsibilities
- Provide direct support with activities of daily living (ADLs), including bathing, grooming, dressing, toileting, and feeding
- Assist with mobility, transfers, and the safe use of adaptive equipment as needed
- Support individuals in following person centered care plans and behavior support plans
- Prepare simple, nutritious meals and assist with eating according to dietary needs
- Accompany individuals to appointments, community activities, and recreational outings
- Encourage skill-building, social engagement, and independence in home and community settings
- Monitor and document changes in physical, emotional, or behavioral status and report concerns promptly
- Follow medication assistance procedures as permitted and directed by nursing or supervisory staff
- Maintain a clean, safe, and organized living environment, following infection control and safety protocols
- Complete required documentation and electronic records accurately and in a timely manner
- Participate in required training, team meetings, and professional development activities
- Collaborate respectfully with families, guardians, and team members to support individual goals
Required Skills
- Personal care and activities of daily living (ADL) support
- Basic knowledge of health and safety practices
- Behavior support and de escalation techniques (entry level)
- Observation and documentation of client status
- Use of electronic health or service documentation systems
- Time management and reliability in shift-based work
- Clear verbal and written communication
- Infection control and universal precautions
- Safe transfer and mobility assistance
- Basic meal preparation and nutrition awareness
- Professional boundaries and confidentiality (HIPAA awareness)
