TITLE: Certified Care Attendant

REPORTS TO: Director of Home Care



Job Summary :
The Home Health Aide/ Universal Worker will provide personal care services as described in the client’s plan of care, in our 24 hour customized living facility. This position is responsible for providing personal care and designated health-related services designed to maintain the client's physical and emotional wellbeing.

Qualifications: Must be active on the Minnesota Nursing Assistant Registry. Must be able to read, write and speak English. Must be able to follow directions and perform assigned tasks. Must be able to relate work with geriatric clients and understand their special needs. Complete and pass a state required background study form which checks for certain criminal convictions or serious/recurring maltreatment.

General Responsibilities:

  • Provide personal care services as described in the client’s service plan of care – including bathing, dressing, grooming, toileting, positioning, transferring, mobility, and any other personal care as described in the service plan developed by the RN
  • Provide delegated nursing services as described in each client’s service plan – including skin care, range of motion, assistance with prosthetic and orthotics, and other treatments described in the service plan developed by the RN
  • Provide homemaking tasks as described in the daily schedule/service plan – including cleaning and straightening of client’s apartments/homes, bedrooms, bathrooms, kitchens, living rooms, cleaning and straightening the congregate living and dining areas, washing, folding, and putting away client’s laundry and linen
  • Serve and/or deliver meals and snacks as assigned
  • Provide psychosocial support for clients by daily interpersonal interaction, assistance with scheduled and informal recreational activities and sincere interest in the emotional well-being of clients
  • Respond to emergency and/or unexpected situation that may arise from response system calls and/or client falls
  • Respond to and report significant changes in client’s health status due to choking, possible heart problems, change in behavior or level of consciousness, abnormal vital signs, hyper/hypoglycemia, abnormal eating or bowel patters, and significant weight gain/loss
  • Review care plan, service schedules, medication services, and program policies with licensed nurse for any possible questions and/or possible changes
  • At each shift change the oncoming and outgoing certified care attendant must complete the narcotic count and complete the appropriate documentation in the narcotic book according to policy
  • Review and ask for additional training in order to adequately provide services described in individual care plans
  • Document daily client care according to policy. Accurately date the charting and initial all cares given
  • Document all significant changes and/or refusal of care Document all new information, changes, useful information, and informal communication in the daily communication book
  • Read communication book daily
  • Participate as a team member to assure client needs are met within a safe environment
  • Attend all mandatory meetings and in-services unless excused prior to meeting
  • Must complete all annual education on RCTC
  • Respect client rights and maintain strict confidentiality regarding clients and their protected personal information in accordance with HIPAA guidelines
  • Is aware of and adheres to the client Bill of Rights
