Talented Medical Solutions is seeking a dedicated and patient-centered Licensed Practical Nurse (LPN) for a Geriatric/Extended Care (GTE) assignment in Rushford, Minnesota. In this role, you’ll be a vital part of a team that delivers long-term, compassionate care to elderly residents who depend on consistency, kindness, and clinical expertise.

As an LPN in a GTE setting, you’ll work closely with residents, families, and fellow caregivers to promote comfort, dignity, and wellness in long-term care.

Key Responsibilities:

Administer medications and treatments as ordered

Monitor patient status and report any changes to the RN or provider

Assist with ADLs (Activities of Daily Living) and hygiene care

Provide emotional support and build trusted patient relationships

Maintain accurate and timely charting

Collaborate with interdisciplinary team members to ensure coordinated care

Active Minnesota LPN license (or eligibility to obtain)

Current BLS certification

Minimum of 1 year of LPN experience, preferably in geriatrics or long-term care

Strong communication and observational skills

Compassionate, team-oriented approach to patient care
