• Provide advanced wound‑care assessments, treatment planning, and ongoing management for complex wounds.
• Conduct routine rounding visits throughout senior living and long‑term care communities.

• Active Minnesota Certified Nurse Practitioner (CNP) license.
• Experience in wound care, geriatrics, primary care, post‑acute care, skilled nursing, or related settings preferred.
• Strong clinical judgment and confidence managing medically complex patients.
• Valid driver’s license and ability to travel throughout the Twin Cities Metro.
