PositionWellness Nurse – LPN (Part‑Time) – Two days a week on Monday and Wednesday.
Job Description- Provide planning and delivery of direct and indirect patient care through the nursing process of assessment, planning, prevention, and evaluation.
- Direct the day‑to‑day activities of nursing assistants.
- Prepare and administer medications as ordered by physician and perform other drug administration.
- Communicate changes in patient clinical condition with physician, nursing supervisor and family.
- Complete required record keeping upon residents admission, discharge, transfer; chart and document per facility policy.
- Customer service oriented.
- Current LPN (Licensed Practical Nurse or RN) license in good standing.
- Thorough working knowledge of current care standards and regulations.
- Experienced in hands‑on care of memory‑impaired residents.
- Can maintain and update effective service plans and supervise care staff.
- Comprehensive working knowledge of current medication regulations and laws.
- Licensed Practical Nurse (LPN, LVN, New Grad LPN, nursing student, or LPN travel nurse/travel contract LPN) looking for a permanent facility with great benefits.
- Experience in long‑term care preferred.
- Can communicate well in both verbal and written forms.
- Committed to interacting professionally and with care toward residents, families, and other staff.
- Graduate or future graduate from an accredited nursing program.
- Hold a current state license as a Licensed Practical Nurse.
- Able to communicate well in both verbal and written forms.
- Minimum of 1 year experience as an LPN in a long‑term care setting.
- Natural problem solver.
- Committed to interacting professionally and with care toward residents, families, and other staff.
