Wellness Nurse – LPN (Part‑Time) – Two days a week on Monday and Wednesday.

• Provide planning and delivery of direct and indirect patient care through the nursing process of assessment, planning, prevention, and evaluation.
• Direct the day‑to‑day activities of nursing assistants.
• Prepare and administer medications as ordered by physician and perform other drug administration.
• Communicate changes in patient clinical condition with physician, nursing supervisor and family.
• Complete required record keeping upon residents admission, discharge, transfer; chart and document per facility policy.

• Customer service oriented.
• Current LPN (Licensed Practical Nurse or RN) license in good standing.
• Thorough working knowledge of current care standards and regulations.
• Experienced in hands‑on care of memory‑impaired residents.
• Can maintain and update effective service plans and supervise care staff.
• Comprehensive working knowledge of current medication regulations and laws.
• Licensed Practical Nurse (LPN, LVN, New Grad LPN, nursing student, or LPN travel nurse/travel contract LPN) looking for a permanent facility with great benefits.
• Experience in long‑term care preferred.
• Can communicate well in both verbal and written forms.
• Committed to interacting professionally and with care toward residents, families, and other staff.

• Graduate or future graduate from an accredited nursing program.
• Hold a current state license as a Licensed Practical Nurse.
• Able to communicate well in both verbal and written forms.
• Minimum of 1 year experience as an LPN in a long‑term care setting.
• Natural problem solver.
• Committed to interacting professionally and with care toward residents, families, and other staff.
