The Registered Nurse Case Manager, RN position is responsible for providing direct care and case management for a team of clients in accordance with agency standards and policies. This position is responsible for performing skilled nursing functions in the home and community setting as well as serving the nursing needs of clients, families and groups throughout the lifespan.

OpportunityDetails
- Targeted pay range: $39.00 - $44.00 per hour.
- Full-Time: 72 hours bi-weekly; weekdays 8:00 a.m. – 4:30 p.m., work every 9th weekend and 1‑2 holidays per year.
- Supported counties:
Dassel, Cokato, Howard, Lake, Montrose, Buffalo, Delano, Annadale. - Pay determined by experience, qualifications, certifications, licenses, and education.

Local travel required.
MinimumRequired Qualifications
- One year of nursing experience in a post‑acute care setting.
- Must be licensed as a Registered Nurse in the state of employment and maintain licensure.
- Ability to communicate effectively both verbally and in writing.
- Proficiency with Microsoft Office Suite.
- Ability to follow all safety rules, regulations, policies, and procedures of the facility, including HIPAA, the Resident’s Bill of Rights, Universal Precautions, OSHA, and other applicable policies.
- Bachelor’s degree in Nursing.
- Previous experience working with seniors.
