Physician Assistant, In-Home Visits

Position Overview

Signify Health, part of CVS Health®, is seeking a full‑time Clinician (Nurse Practitioner or Physician Assistant) to provide In‑Home Health Evaluations. This role involves engaging with people in the comfort of their own homes and bridging gaps in care.

• Bring your heart into every visit, joining a national network of purpose‑driven clinicians dedicated to improving health outcomes.
• Conduct in‑home evaluations that last about 45 minutes, including medication and medical history review, physical assessment, and additional diagnostic tests (e.g., diabetic eye exams, spirometry) as ordered by the health plan.
• Use an iPad and an intuitive clinical workflow for each evaluation; no prescribing, lab ordering, or treatment modification required.
• Travel locally and within licensed states as needed, and conduct virtual visits when appropriate.
• Provide Diagnostic & Preventive Services products such as spirometry, DEE, bone density screening, etc., as indicated.

• Primary workplace is the private residence of our members; environments may include household pets, secondhand smoke, and other potential allergens.
• Candidates must be able to comfortably and safely perform duties in such varied settings.

• Active, unrestricted license(s) in the coverage area(s); board certification is required.
• Multi‑state licenses (preferred).
• Ability to cover the following primary locations:
  Brainerd/Duluth/Minneapolis area of Minnesota, including Carlton, Lake, Saint Louis, Pine, Kanabec, Aitkin, Isanti counties.

Anticipated weekly hours: 40. Full‑time. Pay range: $87,035 – $187,460 annually. The actual offer will depend on experience, education, geography, and other factors.
