RN or LPN - Long Term Care Day Shift

Job Description

Seeking a Registered Nurse or Licensed Practical Nurse to serve at Grace Home, Essentia Health's long‑term care facility in Graceville, Minnesota. This RN or LPN works 6:00 am to 6:30 pm, 36 hours weekly. Every third weekend and a holiday rotation are required. Sign‑on bonuses are available: $7,500 for ASN, $10,000 for BSN (with 2‑year commitment).

• Education as required for licensure.
• Current nursing licensure in the state of Minnesota upon hire.
• Basic Cardiac Life Support (BCLS) certification within 1 month of hire.

Responsible for organizing and providing care to residents through the process of assessment, planning, intervention, and evaluation. Delegates aspects of care to other nursing personnel based upon their licensure, preparation, and job descriptions. Contributes to the mission and goals of the facility and Essentia, and meets the requirements of the Joint Commission if applicable and/or other federal, state, and local regulatory or accrediting agencies.

A caregiver who assumes responsibility and accountability for a group of primarily elderly patients for a designated time frame and provides care to these patients via therapeutic use of self, the nursing process, the environment/instrumentation, and other health care team members. Works both cooperatively and independently, to work efficiently under stress.
