This is not your average NP or PA role. You will serve as a trusted clinical partner to skilled nursing, rehabilitation, and long-term care facilities — delivering the kind of proactive, relationship-based care that actually moves the needle on patient outcomes and reduces rehospitalization. You will have the autonomy of an independent practitioner with the support of an established, mission-driven organization behind you.
What You'll Do
You will provide high-quality primary and/or specialty care to residents and patients across long-term care and post-acute settings. Day-to-day responsibilities include conducting comprehensive patient assessments and physical examinations, managing acute and chronic conditions across a diverse post-acute population, developing and implementing individualized treatment plans, ordering and interpreting diagnostic studies, prescribing medications within your scope of practice, accurately documenting encounters using ICD-10 and CPT code structures, collaborating with facility staff, physicians, and interdisciplinary teams, and proactively identifying high-risk patients to reduce rehospitalization and improve outcomes.
You will see an average of 20–30 patients per day and may travel to multiple facilities within a surrounding area based on patient need. Most providers will have a dedicated exam room, with bedside care provided as needed.

What You'll Need
- Active NP or PA license in good standing
- Board certification by a national certifying body
- DEA license and NPI number
- BLS certification
- New grad NPs and PAs considered for Primary Care roles only
- Specialty roles require prior experience in the specific specialty area
- Strong documentation skills — ICD-10 and CPT coding familiarity preferred
- Valid driver's license and reliable transportation
