Job Title

Key Responsibilities:

• Provide direct patient care, including monitoring vital signs and administering medications
• Assist residents with activities of daily living (ADLs) such as bathing, dressing, and feeding
• Monitor and document patient conditions and report changes to the RN or physician
• Administer treatments and wound care as per physician orders
• Ensure accurate and timely documentation in patient records
• Collaborate with healthcare team members to develop and implement care plans
• Educate residents and families on health conditions and care plans
• Maintain a safe, clean, and comfortable environment for residents
• Follow infection control and safety protocols
