Job Summary:
To provide direct resident care services according to the plan of care prepared by and under direct supervision of the Charge Nurse.
Essential Functions:
- Provide direct resident care services with attention directed toward improving or maintaining optimal physical, mental, and psychosocial functional performance, promoting resident choice and preferences, and increasing resident dignity and quality of life according to each resident's plan of care and the NA Standards of Care.
- Nutritional care, including:
- Asking resident their favorite food and beverage choices.
- Serving resident meal and snacks with correct diet and consistent in a timely manner to ensure proper temperature.
- Assisting, cuing, or monitoring residents with eating.
- Encouraging adequate intake as directed.
- Recording intake as directed.
- Supplying fresh water to residents during each shift.
- Measuring weight for resident as directed.
- Watching for and reporting to nurse changes in resident appetite, intake, and ability to eat.
- Continence care, including:
- Supporting use of bedpan, urinal, commode, or toilet as appropriate.
- Following established bowel and bladder program.
- Collecting routine urine, stool and sputum samples as directed.
- Keeping urinary catheter and tubing properly positioned and cleaned.
- Emptying and recording output for each shift as directed.
- Providing colostomy care as directed, observing for results as directed.
- Recording bowel movements and character as directed.
- Keeping incontinent residents dry and clean.
- Cleansing after each incontinent voiding or bowel movement.
- Watching for and reporting to Charge Nurse changes in resident elimination patterns.
- Mobility assistance, including:
- Assisting with ambulation to the extent needed.
- Using walker, cane, wheelchair, gait belt and other mechanical aids according to the care plan.
- Transferring resident between bed, wheelchair, geriatric chair, or commode using assistive devices, gait belt and proper body mechanics.
- Maintaining optimum joint functioning for activities of daily living by positioning using proper body alignment.
- Moving and regularly repositioning residents who are lying in bed.
- Using proper body mechanics and turning techniques.
- Encouraging residents to be up and as active as possible.
- Conducting range of motion exercises as directed.
- Assisting with rehabilitation and restorative programs and goals according to each resident's care plan.
- Encouraging resident participation in therapeutic activities.
- Assisting residents to and from social and therapeutic activities as direct.
- Watching for and reporting to Charge Nurse changes in resident mobility patterns or safety.
- Reporting incidents or injuries to the Charge Nurse immediately.
- Personal hygiene cares, including:
- Assisting with bathing-bed, whirlpool, or shower.
- Assisting with shampooing and hair care.
- Assisting with skin care needs.
- Assisting with grooming and perineal care.
- Assisting with dressing and undressing.
- Assisting with oral and denture hygiene.
- Assisting with nail care.
- Assisting with shaving.
- Watching for and reporting to nurse changes in resident ADL abilities or skin integrity.
- Comfort and quality of life needs, including:
- With attention directed toward embracing the Culture Change philosophy, make decisions and carry out duties according to the resident's preferred routines and choices.
- Assisting the resident in meeting the needs for uninterrupted rest and naps.
- Watching for signs of discomfort and reporting to the nurse immediately.
- Reporting resident complaints of discomfort to the nurse immediately.
- Following the resident's care plan choices.
- Assisting with socialization, exercise, and mental stimulation of the resident.
- Maintaining a clean, comfortable, and pleasant environment.
- Changing bed linens and making beds.
- Tidy the resident's room.
- Observing of proper ventilation, temperature, light, and noise control.
- Effective communication with residents, families, and other staff by:
- Communicating therapeutically and respectfully with residents by using terminology, manner of speech and non-verbal techniques appropriate to the individual's level of understanding.
- Providing adequate compensation for diminished hearing.
- Establishing eye contact and speaking directly to residents.
- Using low decibel sound levels.
- Decreasing extraneous noise during conversation.
- Responding to resident's verbal and non-verbal expression of needs.
- Reporting to nurse if a request cannot be met.
- Receiving shift report from the nurse at the beginning of each shift.
- Communicating professionally between staff for continuity of care and safety.
- Reporting off to the nurse before leaving duty.
- Watching for and reporting to the nurse and changes in resident usual communication pattern immediately.
- Providing for resident, employee and GSLS protection by:
- Practicing basic medical asepsis to prevent the transfer of infection
