$24.50 - $48.50. Actual pay rate dependent upon experience.

Position Highlights
  • Licensed Practical Nurse (LPN) – Board of Nursing, active license in the state of practice upon hire
  • Varied shifts:
    Monday‑Friday, 7:00 am‑7:30 pm / 7:00 pm‑7:30 am, with every other weekend/holiday; 48 hrs/2 wks
  • Sign‑on bonus up to $15,000 may be eligible
  • New grads eligible to apply
Responsibilities
  • Provide resident care by reviewing physician prescriptions, communicating the nursing care plan and all pertinent information to other members of the health care team, coordinating with other healthcare providers, and providing physical, emotional, psychological, and spiritual support to residents, families, and friends.
  • Develop nursing care plans by obtaining medical histories, examining residents, documenting residents' physical condition, psychological status, and influencing social factors, conferring with family and friends, and establishing nursing diagnoses, care objectives, interventions, schedules, and timetables.
  • Teach healthcare requirements by establishing resident care goals; instruct resident, family, and friends to understand condition, medications, and self‑care skills; answer questions and provide information and resources.
  • Evaluate residents' care by adhering to therapeutic standards; measuring health outcomes against resident care standards; reviewing residents' records; noting changing conditions and needs; re‑evaluating care objectives; modifying the nursing care plan.
  • Maintain a safe, secure, and healthy resident care environment by adhering to infection‑control policies and protocols, medication administration and storage procedures, and controlled substance regulations; adhering to organization standards and policies, and to legal regulations; alerting others regarding potential concerns.
  • Keep resident care equipment operating by following operating instructions and established procedures; troubleshoot breakdowns; maintain supplies; perform preventive maintenance; call for repairs.
  • Keep resident care supplies ready by checking stock; anticipating needs; placing and expediting orders; verifying receipt; stocking items.
  • Coordinate admissions, discharges and transfers to and from the facility.
  • Direct, plan and coordinate workflow of nursing assistants and other nursing personnel.
  • Maintain competence in nursing capabilities outlined by leadership.

The individual must be able to work the hours specified and perform each essential job function satisfactorily, including having visual acuity adequate to perform position duties and the ability to communicate effectively with others, hear, understand, and distinguish speech and other sounds.
