Position Overview
Licensed Practical Nurse (LPN) provides professional nursing care for residents of all ages in assisted living, under the supervision of a registered nurse, advanced practice provider, or physician.
The LPN demonstrates knowledge of nursing principles for the resident in the relevant clinical setting, performs testing to evaluate resident status, administers medications or treatments as ordered, and documents resident interactions. The LPN contributes to resident assessment, provides counseling, and works as a member of an interdisciplinary care team to ensure the health, comfort, and safety of residents.
Responsibilities- Perform testing and evaluate data reflective of resident status.
- Administer medications or treatments as ordered.
- Document resident interactions and outcomes related to care, medication, behavior, activity, and dietary.
- Assist in emergent situations by recognizing problems, recommending solutions, and responding effectively.
- Establish and maintain effective working relationships with residents, health care providers, and the public.
- Communicate clearly, maintain confidentiality of sensitive information, and support interdisciplinary team efforts.
- Counsel residents, family members, and/or resident advocates on prevention and treatment plans.
- Develop self‑care education programs and perform other related duties as assigned.
- Graduated from an accredited nursing program leading to licensure as a Licensed Practical Nurse.
- Current licensure as a practical nurse in the state(s) of practice.
- Preferred: long‑term care, sub‑acute care, or medical/surgical experience.
- Ability to work under supervision of a registered nurse, advanced practice provider, or physician.
