Clinical Nurse assesses, coordinates, plans, directs, implements and manages the needs of assigned patients throughout the episode of care with a focus on progress toward discharge, including during transitions within the acute care stay. The Registered Professional Nurse is responsible for the delivery and coordination of patient care provided by the caregiving team. S/he assures that competent, compassionate patient care is uniformly provided to customers including referring facilities, hospital staff and patients in the inpatient, ambulatory, and community settings.

• Assists the attending Transplant surgeons, house staff, and Transplant staff with the direct care of hospitalized patients and outpatients on a rotating basis with the other clinical care coordinators.
• Facilitation of patient discharges and transplant teaching.
• Attends once a week outpatient Transplant clinics: RN orders labs, performs injection teaching, and coordinates care.
• Schedules appropriate non-invasive laboratory tests and participates in the interpretation of the results and the institution of appropriate therapy based off physician guidances.
• Schedules appropriate surgery and tests for patients.
• Reviews discharge instructions with hospitalized patients.
• Works with hospital case manager and social worker regarding patient discharge needs.
• Answers telephone inquiries from patients and/or visiting nurses after hospital discharge.
• Educates health care workers and the public regarding organ transplantation.
• Participates in teaching programs of the Abdominal Organ Transplant Program.
• Additional responsibilities as needed.

• Monitors transplant recipients after surgery for complications or rejection and reports to Transplant Physician.
• Reviews lab results and biopsy reports.
• Assesses patient symptoms and clinical status.
• Coordinates follow-up visits and routine post-transplant evaluations.

• Monitors drug levels.
• Adjusts medications based on provider orders.
• Educates patient of medication changes and adherence.
• Provides education and support to lifestyle changes, infection prevention, medication schedules, emotional support, resources, and warning signs of complications in inpatient and outpatient settings.
