What You'll Do Complete comprehensive Start of Care (SOC) admissions throughout the Twin Cities Metro. Perform detailed nursing assessments and complete OASIS documentation. Develop individualized plans of care based on each patient's needs and goals. Educate patients and caregivers on medications, diagnoses, and home health services. Coordinate with physicians, therapists, case managers, and the interdisciplinary care team. Help ensure safe, successful transitions from hospital to home.

What We're Looking For Active Minnesota Registered Nurse (RN) license. Strong assessment, clinical reasoning, and documentation skills. Home health and OASIS experience preferred. Excellent communication and organizational abilities. Ability to work independently in a field-based environment. Reliable transportation. Military Veterans Encouraged to Apply We proudly welcome veterans and transitioning service members. Military nursing experience from Army 66H/66N, Navy 1960, Air Force 46FX, Marine Corps medical specialties, and Coast Guard HS is highly valued.
