Overview
As a Private Duty Nurse with Interim Health Care, you will provide one-on-one skilled nursing care to patients in their homes, helping them maintain safety, comfort, and independence where they live. Working within the Healthcare & Medical Services team, you will deliver patient-centered care that may include medication management, wound care, chronic disease management, ventilator or tracheostomy care, tube feedings, and monitoring vital signs, following individualized care plans developed in collaboration with physicians and the broader care team.
Responsibilities- Complete focused assessments and document changes in condition.
- Communicate promptly with families, physicians, and the Interim Health Care clinical team to support optimal patient outcomes.
- Educate patients and families on disease processes, medications, and home care techniques to promote autonomy and confidence in the home setting.
- Prioritize infection control, safety protocols, and adherence to all regulatory and Interim Health Care standards.
- Maintain accurate and timely clinical documentation.
- Build meaningful relationships with patients and families, delivering consistent, individualized care that reflects the company’s mission to provide compassionate, high-quality home health services tailored to each person’s unique needs.
