DescriptionThe MDS Coordinator position provides each resident with professional nursing care assessments and evaluations, is responsible for the MDS process for the facility, is responsible for resident and family education, assists the Director of Nursing in ensuring resident care policies are carried out, and assists in planning, organization, developing and directing the day-to-day functions of the Nursing Services Department in accordance with federal, state and local standards, guidelines and regulations that govern our facility, and as may be directed by the Administrator, Medical Director and/or Director of Nursing to ensure that the highest degree of quality of care is maintained at all times.
Position RequirementsThe MDS Coordinator is required to have a current RN license to practice in the state of Minnesota and current CPR certification, required to have a minimum of 2 years of long-term experience, must have working knowledge of the MDS process, ability to demonstrate leadership abilities to develop and motivate people, process good critical thinking and problem solving skills, possess the ability to prioritize responsibilities, be flexible and self –directed, working autonomously having the necessary management skills to plan for and oversee the utilization of resources.
Practices highly sensitive interpersonal communication skills with staff, families, physicians, other departments and residents. Must also have the ability to create and maintain an atmosphere of warmth, personal interest and positive emphasis as well as a calm environment throughout the facility, must be able to cope with the mental and emotional stress of the position relating to and working with the ill, disabled, elderly, emotionally upset and at times hostile people within the facility.
Must be able to read, write, speak and understand the English language.
- Individually responsible for MDS from start to finish including accurate documentation, completion in allotted time frame, and submission when required.
- Acts as a coach to nursing staff to keep the process resident – centered.
- Develop and maintain a good rapport with all services involved with the care plan to ensure that a team effort is achieved in developing a comprehensive plan of care.
- Ensure that nurses’ notes reflect that the care plan is being followed.
- Ensure all personnel involved in providing care to the residents are aware of the care plans and that care plans are used in administering daily care to the resident.
- Review resident care plans for appropriate resident goals, problems, approaches, and revisions based on nursing needs.
- Direct the nursing staff to obtain the appropriate data for individual resident’s bowel and bladder status. Analyze the data to determine resident’s need for further B&B programming.
- Performs AIMS assessments with the MDS processes.
- Participate in making recommendations of residents that may require psychological services to the Director of Nursing.
- Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary.
- Participate in surveys (inspections) made by authorized government agencies.
- Evaluate accident/incident reports as required. Assist the Director of Nursing in analysis of incident reports to determine cause and appropriate action.
- Transcribe physician’s orders to resident care plans as required.
- Report all discrepancies noted concerning physician’s orders, diet change, charting errors, etc., to the Director of Nursing.
- Provide data to the Quality Assurance & Assessment Committee.
- In the absence of the Director, serve on, participate in and attend various committees of the facility as directed by the Director of Nursing.
