Job Summary

Registered Nurse (RN) responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care to patients/residents. Collaborates with patients/residents and families, and inter‑disciplinary colleagues, including providers, to plan, implement and evaluate care.

• Utilize the nursing process (assessment, diagnosis, planning, implementation and evaluation) to provide individualized nursing care.
• Collaborate with patients/residents, families and providers to plan, implement, and evaluate care.
• Demonstrate competency within the full scope of nursing expertise and apply appropriate age and population‑specific standards.
• Provide care for patients/residents in all phases of preventative care, health maintenance, diagnosis, treatment and follow‑up in the home setting.
• Coordinate care, conduct assessments, educate patients/residents and perform various nursing interventions.
• Serve as charge nurse when needed and delegate tasks to licensed and unlicensed assistive personnel within scope and competence.
• Provide assistance and support to delegates to ensure safe, reliable administration of care and resolve care and service concerns.
• Operate within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules of the state of practice and licensure.

• Graduate from an accredited nursing program (AACN, ACEN, NLN CNEA);
  Bachelor’s degree preferred.
• One year of long‑term care, sub‑acute care or medical/surgical experience preferred.
• Current unencumbered RN license with the State Board of Nursing where practice occurs, and/or multistate licensure under a Nurse Licensure Compact state.
• Obtain and maintain required certifications, CEUs and competency training.
• Ability to adapt to age and population‑specific standards.

Shift: Day (United States of America).

Hours:

24 hours per week.

Compensation:
Salary range $39.00–$48.00 per hour plus $10,000 sign‑on bonus available.
