Gundersen Health System (Emplify Health) – Harmony Care Center is a long‑term care facility dedicated to promoting a healthy, meaningful and secure environment for adults of all ages, and we are looking for a Certified Nursing Assistant to join the team and work PM shifts.

• Work PM shifts including every other weekend and holiday
• Part‑time – 32 hours/week
• Nurture a family‑based care environment by learning about each person and serving their needs
• The CNA is responsible for performing basic care of patients under the general supervision of an RN in the acute hospital setting
• The CNA is also responsible for transporting and transferring patients within the medical complex in a safe and efficient manner and assists with data collection to facilitate delegated nursing care

• Work closely under the supervision of a licensed nurse
• Must be physically able to lift heavy objects, as well as push, pull, reach and squat on a regular basis
• Accurately document each resident in an electronic medical record and must be familiar and competent in basic computer use
• Assist with basic care for patients in the areas of nutrition, elimination, comfort, mobility, and safety
• Ensure patient safety
• Participate in CNA meetings and unit staff meetings
• Participate in quality improvement activities to continuously improve patient care and unit operations
• Perform other job‑related responsibilities as requested by the Director of Nursing
• Must be flexible and adhere to regular and predictable attendance
• Transport and transfer patients within the medical complex using appropriate equipment and safety precautions as indicated by the patient’s health status
• Assist nursing staff in lifting and ambulating patients using appropriate practice guidelines
• Maintain environment and equipment
• Demonstrate dependability, organizational skills, and effective use of time by setting priorities to complete delegated work within established time frames in accordance with unit guidelines
• Demonstrate role accountability by adhering to Gundersen Health System mission statement, policies, and confidentiality standards

• Minnesota Registered Certified Nursing Assistant (CNA)
• If you do not have your CNA certification, we will pay for it and can start you as a Nursing Assistant
• Current Infant‑Adult Basic Life Support for Health Care Providers (with hands‑on learning) from American Heart Association or American Red Cross

• $20.55/hour + additional pay for experience
• If you do not have your CNA certification, we will pay for your training and if starting as a Nursing Assistant, the pay rate will be $18.57/hour until you are certified
• $2,500 sign‑on bonus after 6 months of employment for CNAs only (taxes apply)
• A welcoming team environment supportive of new and current staff
